Overview of Use of Funds: Good Works Grant Writing is excited to congratulate our client, Elaine, for being selected to receive a $25,410 grant from the Blue Cross Blue Shield (BCBS) of Arizona Foundation! This generous corporate grant will support Elaine’s Transportation Assistance Program (TAP), which serves homeless, elderly, disabled, low-income, and other vulnerable people age 18+ in the Valley in response to an urgent community need for transportation to locations essential to the Social Determinants of Health (SDOH). Through TAP, Elaine serves vulnerable people in need of transportation within a 15-mile radius of the Human Services Campus in downtown Phoenix, providing free rides so clients can access services that help them make improvements to their SDOH. Funding from BCBS will allow Elaine to 1) Provide 546 free rides, 2) Serve 150 unduplicated individuals, and 3) Drive clients to 338 critical SDOH locations.

Overview of Grant Objective/Impact: Elaine’s Transportation Assistance Program (TAP) is the nation’s first and Arizona’s only program that addresses equity of access to all SDOH. SDOH services/locations include: mental/medical healthcare appointments, food access, job interviews, housing Interviews and lease signings, criminal justice/case management appointments, economic opportunity/Department of Economic Security and bank visits, social and cultural cohesion/community centers and support groups, and critical errands (laundry, mobile phone providers, etc.). Without TAP, individuals needing services not covered by Medicare/Medicaid or other traditional funding sources have no other free, reliable, punctual alternative for getting to healthcare and social services that are available to them. With TAP helping Elaine clients access these services, Elaine is helping our community’s most vulnerable to make significant strides in improving their health and well-being in the short- and long-term.
