Overview of Use of Funds: Funding will help bolster Banner Health’s senior SNAP benefits enrollment program at the Banner Olive Branch Senior Center in Sun City. With a $50,000 grant, Banner Health will extend Olive Branch’s current benefit services to other Banner Health locations beyond Phoenix’s West Valley; train staff/volunteers on program policies; train our front-line personnel (social workers, home health aides) to meet with seniors during their discharge/appointments, engage in one-on-one discussion regarding SNAP eligibility and help submit applications; and conduct follow-up for benefits usage and recertification.

Overview of Grant Objective/Impact: The Banner Health Senior SNAP Benefits Enrollment Program, with the Banner Olive Branch Senior Center as its hub, will serve seniors in the greater Phoenix metropolitan area/Maricopa County, the greater Tucson metropolitan area/Pima County, and Payson, Arizona. By addressing food insecurity at the points of care, Banner Health aims to treat our patients and members holistically, creating the conditions for better health outcomes and management of diet-related chronic medical conditions.
